Family Navigator (Temporary Full-Time)

Ahtahkakoop Cree Nation Health Centre is seeking an enthusiastic individual with Excellent interpersonal communication skills and the ability to work as a team player to effectively interact with community, clients, departments and staff, elected officials, members, service providers and local government agencies and officials The Jordan’s Principal Family Navigator will be responsible for helping families access available funding through Health Canada, to address unmet health needs of children up to the age of 18. He/She operates in an outreach environment, such as in homes, schools, and/or community agencies. This position requires a broad skillset and the ability to work cooperatively and collaboratively with all Ahtahkakoop Cree Nation departments, the broader community and Urban Systems. Our ideal candidate should also bring a genuine interest in learning more about community planning and a passion for improving our quality of life at Ahtahkakoop Cree Nation.

Roles and Responsibilities:

·         Build relationships with community resources and collaborate with existing community based programs and services

·         Enhance awareness of existing programs and available supports for First Nations living on and off reserve

·         Work with community and service providers to proactively identify children with unmet needs to facilitate early intervention and timely access to services and supports

·         Build case management capacity through development of policies, procedures and protocols related to screening and assessment, service planning, monitoring and evaluation of service plan

·         Facilitate access to professionals for assessments or provide professional services where/when required

·         Undertake follow-up with clients/families and key contacts to ensure the chi d is receiving and maintaining the services required

·         Identify and work collaboratively with federal, provincial, territorial, regional and community partners to implement promising practises and evidence-based models, service arrangements and supports where possible

·         Ensure culture is reflected in care where First Nations people are treated with respect, compassion and cultural understanding and assist to build cultural competency within the region and broader health and social system

·         Collect information and support case coordination to ensure seamless transition of cases when required

·         Knowledge of Cree culture, language and social/health issues is an asset

Required Qualifications:

·         Bachelor degree in health science, education or related profession e.g. administration, communication Three (3) years of relevant experience in case management, program development and implementation for children and youth in one of the fields related to Jordan’s Principal: medical, health, social, special needs, education

·         Knowledge of available federal and provincial programs to support children, youth and families

·         Strong oral and written communication skills

·         Ability to conduct research, ability to deliver presentations to different audiences (professionals, families, youth)

·         Exceptional organization and time-management skills and ability to prioritize multiple tasks

Highly motivated and ability to work in demanding and fast paced work environment
